A reminder from the
state of Illinois: ALL current Kindergarten, 2nd and 6th grade
students must have a 2012-2013 dental examination form on
file at school by Wednesday, May 15, 2013 in order to receive their end of the
year report card. Letters have
been sent to those parents whose children are missing this form. Please take care of this as soon as
possible. Your child’s dentist can fax
the dental examination form to school. The
2013-2014 Health Forms - Sports Physical, Medication, Dental and Food Allergy forms will be sent home with your child’s report card on the last day of school. Please call me if you need a form before the last day of school.
Keep up the good work making sure that your child is getting a good night’s rest and eating a healthy breakfast before coming to school each day. Continue to pack a healthy snack and lunch for your child. Encourage outside play now that the spring weather has arrived. And, of course, keep your child home whenever they are ill. I tend to see more skin rashes at this time of year. Please do not send your child to school with a rash without a doctor’s note.
