The end of the school year is fast approaching. I am trying to get all the required forms collected. For all current Kindergarten children to receive their report cards at the end of the year, I need their dental and eye exam forms turned in. All second graders and sixth graders need to turn in their dental forms in order to receive their report card. Please turn them in as soon as possible. All children who failed their vision testing in school received a form home to take to an eye Doctor to check their vision. Please return those forms also as soon as possible. I will be sending home the forms for all children entering second and sixth grades for dental and physical exams. Pleases have them done over the summer and return them at registration or the first day of school. On this web page you are able to download some of the forms needed. I will also send home the forms for your child to have medication in school. I will send it to he children that need their inhalers, epi-pens, or other medication that I know of. If you need one and don’t receive one, please contact me and I will be happy to send you one. It has been a good year that has flown by. I hope everyone has a healthy and safe summer.
