Registration Information
You must
complete the registration process before your child can attend
All
of the items listed below are required by District 80 before you may begin
filling out registration papers for your child.
Please have them ready for us to Xerox for your child’s file:

Original Birth
Certificate
To
verify that you are a legal resident please provide the following:
v
Tax Bill,
Mortgage Statement or Signed Lease
v
Gas, Electric or
Phone Bill
v
v
State of
If you are living with a family within our
District you must submit a notarized District Statement of Residency form and
provide all of the above information pertaining to residence from the
homeowner.
